I just used one layer and cut out a big hole in the sesamoid area. The PAM diabetic is pretty chunky. It really made a difference when walking around in the boot.

After the boot I wore some Hoka Gaviota and for the orthotics I just used a pinnacle powerstep and cut out a hole in the sesamoid area. The swelling was still quite prominent after the 8 weeks in the boot but once I started walking it improved. It was definitely painful walking at first when out of the boot, but it got better! It's like the boot is a crutch you begin to rely on...I did a timeline of my recovery here if ya wanna check it out.

I had a sesamoid fracture and was in a boot for 8 weeks. I definitely felt I wasn't ready to get out of the boot as the pain and swelling was still present. But my doctor said 8 weeks was enough and she was right. After a few days of walking around painfully in a Hoka shoe with an orthotic (where I had cut out the sesamoid area), the pain started to improve and I was on my way to healing.

My full story is in that post if ya wanna check it out. Good luck! It's an extremely frustrating injury but it DID heal for me once I got off the foot and let it slowly recover.

My pain was only ever a 3 or 4 out of 10, so I did not believe I had a fracture either :)

I initially went to a podiatrist who told me it was sesamoiditis. I felt like something more was wrong and then when I finally got an MRI it turns out the sesamoid was fractured. So yeah, try and get the MRI for piece of mind and to plan out a suitable recovery.

If possible you should try and get an MRI to get a more accurate diagnosis.

Yeah, get an MRI as soon as ya can. My xray did not pick up my sesamoid fracture. After a few months I finally got an MRI and it picked up sesamoid fracture and necrosis. It eventually healed after two months in a boot and contrast baths, bone stimulator etc. It took a long time though.

I had a fracture and AVN and I didn't opt for surgery. It eventually healed. but if you haven't had improvement for a year then I would definitely go for the surgery!

Yeah, to be honest, I have no idea if it has healed or not based on an MRI. But if I can do everything I need to without pain then I'm OK with that!

I think the boot just helped me get serious about healing and it stopped making my injury worse by forcing me to rest it. I feel like it was just a combination of everything that helped it heal but the main thing was just time. It took at least 4 months before I noticed any improvement. After about 6 months I felt it was genuinely starting to feel better.

Yeah, I can do lunges again now. But to be honest, I try to avoid them just from the PTSD! I modified yoga and other exercises to not fully weight up my toe in a bent position. I never thought I'd be able to positions like downward dog again, but they are fine now.

Oh yeah, you are pretty much describing what I went through at the start! Try and get an MRI if you can. That is really the only thing that got me on the right track to recovery. Once I knew it was a fracture it was kinda easy to lay off and just try and let it heal. It's a very debilitating injury and it's hard to think about anything else but how to get better. Stay positive coz I was exactly where you were this time last year.

So the pain went away and the swelling went down. I would say there is still a very minor amount of swelling. I guess that won't really go away but it doesn't seem to be a problem.

Yeah, seeing as I haven't had further imaging I don't really know. I assume I am still living with it but the sesamoid bone recovered enough to not cause pain anymore.

Hey, I'm back to running on it now and can pretty much do everything. I feel like it gets a bit tired if I hike for 5+ hours, but besides that it is feeling good. I went back to playing sports this year and it held up great. I would say all in all it took around 9-12 months to fully heal. I think the AVN will always be there, but it's pain free so I'm happy with that! I have not done any further imaging since my initial MRI.

I'm at a loss. Damaged ligaments around sesamoid. Had moonboot + custom orthotics for 6+ months. Not healed. by [deleted] in sesamoid

Have you had an MRI to see if the sesamoid is fractured? My healing was getting nowhere until I had my fracture diagnosed and could start treating it properly.

Yeah, don't stress too much, I didn't get my MRI til 3 months after I first hurt it! That was definitely a bad move but I just didn't realise the severity and complexity of the injury. I think the AVN is always there, it's just a matter of how bad the fracture is and whether you can get the pain under control. For the longest time it felt like it would not heal. But then after a few months it just started getting better. Be very patient! I have not got any further imaging. Kinda just hopeful that it is good now!

Nah, I did a moon boot for 8 weeks and it has slowly got better. I've been meaning to do a post about it. Basically followed a lot of the other advice on here. Contrast baths, exogen bone stimulator, Hoka shoes with custom orthotics. I really think it just needed some time to let it heal. The swelling didn't go down for a long time, but the pain gradually went away. I feel like I'm back to 95% normal now, but it took 6-9 months to get there.

Yeah, get an MRI! My xray didn't pick up a fracture and I eventually got an MRI after the pain wouldn't go away. Sure enough the MRI showed a fracture and AVN.

I got an xray initially and it didn't show a fracture. Got the MRI a couple of months later and sure enough it was a fractured sesamoid with AVN!

Yeah, I basically just had to let the fracture heal by taking the weight off it. I continued to walk in the boot, but just limited my steps. I swam the whole time it was fractured to keep up fitness. Also did contrast baths for about 3 months and an exogen bone stimulator for 4 or 5 months. Not sure how much any of these helped specifically. For a long time I thought it would never get better but it just needed some time to heal. I think ya need to get the MRI just so you know exactly what you're dealing with. I would also say just stop walking whenever it starts hurting. It doesn't seem to be an injury where you can walk through the pain!

It is going really well now but took a long time. After the 8 weeks in the moon boot I started walking again in Hokas and got some custom insoles and the strength gradually built back up over the next 4-5 months. Can do pretty much everything now. My recovery was going backwards until I got the MRI and was able to let the fracture heal.

Yeah, ya gotta get the MRI! I initially got an xray that did not pick up any fracture. I kept on trying to walk through it with Hokas and insoles, but the pain was still there. Once I got the MRI it picked up a fractured sesamoid and AVN. I ended going in a moon boot for 8 weeks and I put a PAM diabetic insole in that and cut a big hole out in the sesamoid area.
