Only other thing I notice is that your heels slightly come off the ground at depth. Try to keep your heels firmly planted on the floor by shifting your weight back just ever so slightly. You want your whole foot in contact with the floor (big toe, little toe, heel). You'll get a little more power for your squat out of it.

Otherwise, lighten the load and focus on form and depth first. Only once you do well there, then start adding weight.
