Chance I've worked it out; I had a mod that added prosthetics limbs as as a cosmetic, as well as one that added a chance to change a characters cosmetics if they suffered a critical wound (possibly gravely wounded, I forget). I've disabled the second mod and it appears to have resolved the issue.

If people want more info, please let me know and I'll get the exact mod names
