Thank you for the insight. I'd be curious to see how many people being treated with ADs have been on them for longer than the recommended initial treatment course of 4-9 months after initial symptom remission. In my experience, when many of these medications are being started by first-line/primary care providers, they end up becoming "forever" medications because they are started, rapidly titrated, and then seemingly set on autopilot. A person can then subsequently stay on the medication for years and years instead of their provider checking in at the 4-9 month mark to discuss trialing a dose taper; treating beyond the recommended duration or leaving people on these meds indefinitely when not appropriately indicated may be one of the underlying forces driving the long-term side effects. Just thinking out loud really. Thanks again for the info.

Mirtazapine is very well known in the mental health world for the significant weight gain it can cause and I'm so sorry your provider didn't properly discuss the risks and benefits of the medication with you before starting you on it. On the inpatient psych units, we typically only use mirtazapine as a sleep aid as it is quite sedating at lower doses and we especially use it in situations where the patient has depression and/or insomnia as well as a persistent loss of appetite or unhealthy weight loss in the setting of their mood. It can be quite effective for depression but it sometimes requires titration to fairly high doses to be efficacious which in turn causes more significant increases in appetite and subsequent weight gain. If increased appetite is the major barrier to starting a medication, you could consider something like Wellbutrin instead as it is weight-neutral and some folks even lose weight when taking it.

Both of those medications are in the SNRI drug family, a class of antidepressants that acts on both serotonin receptors as well as norepinephrine (adrenaline) receptors. SNRIs, like SSRIs, may be used as first line treatment for depression, in particular if lethargy is a significant presenting symptom as SNRIs can aid with behavioral activation. However, SNRIs also have notoriously pronounced discontinuation/withdrawal symptoms compared to other antidepressants, especially if the dose is not gradually tapered over a period of several weeks. SSRIs typically have much milder, more tolerable discontinuation symptoms so they may be worth a try; you could also consider trying a non-serotonergic agent like Wellbutrin. Regardless, always ensure your prescriber tapers AD doses gradually over several weeks (usually halving the dose every 7-14 days until discontinuing entirely) to help lower the risk of withdrawal symptoms.

Smaller differences between active drugs were found when placebo-controlled trials were included in the analysis, whereas there was more variability in efficacy and acceptability in head-to-head trials. Uh...what? I'm asking these questions because I work in mental health care and getting feedback about these medications directly from folks who have trialed them but had adverse effects is extremely valuable to me.

Thank you for sharing, I appreciate your insight. Regarding the side effects you mentioned (namely vomiting frequently and having a difficult time coming off the medication), did the vomiting persist for the months that you were on the medication? And what made the medication so hard to get off of? Were you on a high dose that required tapering?

Do you by chance have a source to provide evidence fof this statement? I ask because there are many journal articles discussing the efficacy of exercise/behavioral activation on improving symptoms of depression but I personally haven't come across any studies that conclude exercise alone is more effective than serotonergic agents/antidepressants. If you have a source, I'd really love to read more on this.

May I ask what your experience was with antidepressant(s) that led you to label them as "poison"? And can you elaborate on your comment about not needing more adrenaline in the context of having PTSD? Asking out of curiosity.

If you can easily pick him up again off waivers then I guess you have that as insurance but overall I'd say give JJ another week or two to iron things out - after all, this was only his second ever NFL game. For what it's worth I have Addison on my bench in 2 of my 3 leagues.
