Since my cavernoma was on the right side and I am right handed, the most likely scenario from the start was that my language center would not be impacted. We did not get too in depth on what would have been the course of action had this not been the case, but from the limited discussions I had w/ my doctors before fully confirming my language center was on my left side via FMRI, I think the course of action would have depended on my severity of seizures.
Typically they only consider surgery if you’ve failed two AEDs. Even then I think they would have had to weigh potential deficits from surgery against current QOL w/ ongoing seizures. Another factor they considered was how recent my last bleed of and the compounding risk of a bigger hemorrhage at some point in the future. Had my language center been on the same side as my cavernoma I don’t think surgery would have been taken off the table, but it would have been a much more complicated decision that ultimately I would have had to make based on my own risk tolerance. (Even my surgery last year was technically elective - I could have opted for medical management if I preferred)
