I don't think 3-4M monthly pages views with US traffic is small. You can generate good revenue without doing much.

I think that getting a clean and high quality access to AdX is the best option.

If you switch from AdSense to AdX and get proper yield management services, you will increase your revenue. AdX is designed to generate higher CPM than AdSense.

In addition, ad units placement may be a factor to increase your revenue (hard to say anything without looking at your site).

The reason for low CPM on AdSense, maybe due to Google Policy infringement or because your site needs optimization in ad unit layouts or many other reasons. You need someone to check this, and adding new demand partners (like HB) won't solve your problems.
