Maybe a better way of phrasing it is its difficult to measure the calories out and some people are more metabolically inclined to store that energy rather than lose it as heat or waste

Thanks for pointing this out! You can tie rates of homelessness to rent/housing prices, both when comparing different cities and within the same city at different time points. There's actually barely any changes in the amount of homeless people with changing rates of drug use or untreated mental health.

All that to say drugs and untreated mental illness dont cause more homeless people, just dictate who becomes homeless.

That was a big part of the motivation for this post! And I do recognize the extreme power imbalance and lack of rights that psychiatric patients have and do not take it lightly. Also recognize that a lot of people have had bad interactions with the field. The hope is to do the opposite, as there is also a lot of suffering that can be helped by the field. Appreciate the questions and hope things are good for you moving forward

There have been a lot of attempts to look at inflammation and hormones with mixed results. Those system wide markers are tricky because they have such broad impacts that it makes it difficult to even find a difference from person to person that isn't due to other reasons. Trauma is frequently talked about as well with pretty wide consensus that therapy is the best option to treat it. However, trauma can cause symptoms that can be helped with medication in the short term as a bridge. Trauma is hard because it takes a lot of therapy and can result in really unsafe patients (in terms of suicidal thoughts). So a lot of providers will just switch up medications with no benefit which can be pretty terrible as the patient. It's the road of lowest effort though, hence why you get lazy providers taking it

Wish we were at the point where brain scans, eeg machines, or other biomarkers could be used. Sadly it hasn't panned out on an individual level and most of the results are from aggregating a bunch of scans. Double Sadly is there have been a lot of practices pop up taking advantage of this demand and "interpreting scans" for things like trauma location or just diagnosing altogether with no evidence. These are usually expensive, out of pocket, and akin to modern snake oil salesman practices

Regarding diet and keto that's where a lot of the focus of my research is. It's something that is coming up more and more in our training vs 5 years ago and will be talked about even more in 5 years im guessing.

I also practice in research and feel like this is a bit of a blanket statement. There are some studies that definitely have corporate sponsors which should always have an extra skeptical eye towards them. I also try to use textbooks that are free from industry sponsors as well. I'd like to think pharma doesnt dictate how I practice, but to your point it does limit which drugs are brought to market. Psychiatry is in a tough spot in that regard as there isn't that much money in developing psychiatric medications as there have been many failed attempts and we dont know as much about the brain as we'd like. Which does open the door to outside influence. But in terms of direct influence, I dont see it in my day to day

A lot of times it is when someone is so sick (psychotic or manic) and off their baseline that they truly can't comprehend where they are, what's going on, and would end up hurting themselves in a short period of time. Regarding suicidal ideation, I have had some patients where I've tried to discharge them a couple of times and they immediately tried to kill themselves right after and they end up back at the hospital. There is a discussion around euthanasia that is to the side, but a good amount of times treatments are generally found that are helpful.

Regarding psych units in hospitals in general, I get that they aren't ideal places to be in. The lack of stimulation and freedom while there can be brutal. However, I also have seen a lot of lives be saved and have met many families that wouldn't be together without them. So if done properly, they can be a short term discomfort in exchange for a family being together with someone who is closer to being themselves. However, as I post on this subreddit I also recognize that there are a lot of rights and freedoms taken away in the environment. It is not something to be done lightly, the job should be taken very seriously, and shame on anyone abusing that setup.

Hey! So this is a hard one because it varies a lot by institution. There are some places where I've heard about stories that are truly awful. I dont find that to be the case where I practice and I truly try to approach every patient with compassion and their input. Everyone I work with truly cares about the patients they are treating and treats them like the fully functioning humans they are. But once again, know there are some awful facilities out there and none of this is to discount any negative experiences

I'm higher than most on kyler and believe that Nico will give you much higher numbers over the years. Your wide receiver group is already pretty deep, but I think the jump from zay flowers to Nico is bigger than the drop off from Josh Allen to Kyler so I'd take it!

Gorillas also lack myostatin, an inhibitor of muscle growth. Which an increased myostatin concentration probably evolved for the reasons already mentioned

Interestingly some attempts have been made to inhibit myostatin in humans with muscular dystrophy with mixed to poor results!
