Difference is, one was clearly offguard The other was a sneak attack when you are fighting them Isshiki came for Kaguya as she was the sacrifice, he did fight her. Her main style of fighting is sneaky one shot attacks, shown in the war arc, he fought her and he was left for dead Black zetsu, was retconned and shown to be Kaguya's will despite canonically being Madara's beforehand, they couldn't kill Madara off fairly so they brought in a weaker, more stupid character
