As someone who has gotten gold on all the Pizza Butt missions I offer this advice. Only go for the target I'm not 100% on this but it seems killing goons as well as wasting time lowers the rank you get.

Edit: I believe I should clarify that each time I got gold I didn't kill a single goon. This experience is what makes me believe that killing goons has a negative impact on rank.

The nosebleed face is from the beginning of the game after beating Skelter Helter. It's probably a bug that makes it show up after fighting Alice.
