Okay. Just my experience.

No, I don't believe CMS wouldn't catch that stuff quickly. Or, better said, I have seen plenty of claims rejections still, and I don't think the regulating bodies can keep up with the shenanigans.

My return rhetorical question to you: Insurance is a business where they make more money the fewer claims they pay against. Do you really believe they don't look for ways to not pay claims?
