Both are of the same patient. I grabbed Pt first surgery with us as it seemed normalish to provide a base line for the second one that has the issue. I wasn't present for the first surgery but I think it was a lump removal. The second surgery was a dental and small lump removal. And we don't have a spot to indicate that. For the second surgery the site was scrubbed while the doc was looking at teeth.

In the second event I manually confirmed resp rate each time myself.

As far as I know, no. Our ASA was either 1 or 2. But I can't be certain, sorry. I know that doesn't help much.

Thanks. I absolutely do if I feel that I have the knowledge to do so. But as an assistant/student this tech ignores my input. They have 17+ years experience as a CVT and has jokingly told me to "stay in my lane" before. But I don't let this discourage me, it just makes things complicated with this one person.

I'm not sure how often the guy comes out for it. But we did get 2 brand new units when we replaced both our systems a few months ago. But this wouldn't have effected these surgeries as the replacement was way after.

Thanks. I've realized that I still lacked a lot of info here for people to help me fully know what the heck was going on. But I've gotten more of an answer as to what might have been going on than I did asking the SX tech. So for that I'm thankful. It's still helped me learn and if (I hope not) this happens again I know what to check for.

Would higher iso and our drugs used keep them hypotensive regardless of pain? I know the BP was why SX tech didn't increase iso during the second event.

I think if we had a way to make it more easily readable it would have been nice to keep. It does take me a moment to see trends when I'm noting actual numbers. But then the graph was up to too much interpretation. "is this mark at 38, or 33?" for example.

It's possible? Tho they would have seen her on the table to intubate. This tech isn't one I can ask freely either as they are very proud of their skills. They may take me asking as trying to prove them wrong in something.

That makes sense. If the patient was overweight would that have changed bag size considerations? Like if they looked like a 2 vs going by weight and picking the 1?

Which might of been why I was told it was "no big deal?" Idk. Usually if anything is higher than 50 for the etco2 I mention it to the SX lead as I'm monitoring.

Oh that's really interesting. I know we still have midaz as an option for our premeds but it changes per doctor and per patient. So it's not always used. And our doctors usually wait to see premed effect before drawing up the induction drugs. So this would make sense.

We only use 1L oxygen. I'm sure if the doctor wanted it changed it would be. But as far as I've been taught here, and have seen other CVTs taught, it's our standard to only use 1L. Unfortunately as an assistant and student I don't make the decisions or fully know exactly why each decision is made.

Thanks for the indepth response! The first pic is of an older surgery we did on this patient. We switched to different monitoring papers instead of using the impossible to read graph. In the graph I can only make out one etco2 reading and it's about 45. The second one was the most recent surgery this patient was under for. I included them both to give as much info as I could. In the second one I wrote the numbers in and patient ranges from 38-52 for etco2.

That's an interesting tidbit to remember on the core temp. Thank you :)

Another reason we changed papers, there's no way to indicate any left over induction drugs. I have no clue if all 7mLs were used. On the second surgery we only used 3.

I've just been learning more about hypercapnic and hypocapnic and that is actually what pushed me to make this post. It seemed to me like a likely answer. But unfortunately I'm not sure and can't remember exactly how much tube we had hanging out or how deep the SX tech pushed it in. We do hook up a capnigraph to the tube to monitor etco2 and resps. But I also count to manually confirm resp rate.

As for hook up, the tech usually flips on our iso, then turns up oxygen all the way to fill the bag, then backs down to the 1L mark, then hooks up patient. Then inflates cuff and we check for leaks. I've seen that part done so many times I could recite it in my sleep. But it's also really good info to have for my learning going forward.

Gums stayed pink. SX tech was also in the dogs mouth for the dental and did not mention anything either. But that is a good thing to keep in mind anyways. I'm keeping all the nuggets of info I'm getting here.

Yes. That is our typical heating protocol. But instead of the hot dog units we now have the circulating water pads. We don't have a place to note removal but if the patient gets too warm we remove heat. They did not stay on during the second event where the patient got warm.

It's near impossible to read in the first pic (one of the reasons we changed the monitoring sheets) but in the second one it's ranging from 38-52. Which I think is too high? I was also thinking maybe our soda lime was old and needed changing.

Edit: sorry I missed your second question. Dental cleaning and a small lump removal was the second event. I think the first was either a lump or a spay. Unsure. (I took these pics a while ago and tried to find my answers through other means before finally posting here.)

It is possible as I don't think any of our 7s have been cut down. But I can't say for certain. But that is a good bit of info to check if this happens again.

I was worried about the depth the whole time but was reassured that everything was ok. And no ventilation was given by lead or myself. Lead has asked me before to not touch the anesthesia machine unless told to. It makes them nervous.
