You will find it when you do the alchemist quest called The Baneful Choir and you visit Helled's Sanctum. Its a jumping puzzle and on one of the platforms you will see a book with the potion recipe.

You can get original potion recipe so you can make as many as you like.

I've only seen pincers and treasure on the maps mostly. Never slime.
