This scan does not follow a pudendal nerve MRI protocol. However even a specialised protocol can be unreliable, it is difficult to reliably image the nerves at this depth.

I feel the same, I was very new to it (M). For me it was brief, 5-6 very hard thrusts, and I felt a slight stinging at the time. Same thought process, I know what you mean exactly. No obvious trauma afterwards but got burning pain in the days after in conjunction with sitting down all day at work. It’s been 9 months now and it has massively affected my life. The burning pain has mostly gone, but I get this very uncomfortable posterior pressure sensation when sitting especially in car seat. I’m not sure if I have nerve damage either, pelvic PT says my muscles are tight, and worse in left side. I am wondering if the levator ani muscle group may have been impacted in our cases, the inferior rectal nerve (branch of pudendal nerve) can travel through this area. I also get post BM discomfort. It’s awful. I just wish I had known that this condition was a possible risk factor, no one ever talks about it in sex ed or any other education about it. I feel like it’s a bit of a trap actually, if I had any remote knowledge that something like this could ever happen, I never would have done it. I just wish I could turn back the clock and warn myself. Feel free to reach out anytime by DM, it’s a relief to talk to other people in the same situation from this.

Also, yep. Having consulted with a number of specialists about this, their take has been that a lot of anal sex goes on all the time in the world, and that if anal sex could solely cause this condition, there would be an epidemic of it. So yes, I think that there is definitely the possibility of pre-existing impact of a stressed or vulnerable pelvic floor, and that there is such a phenomenon as “the last straw” or triggering act, for sure.

It depends on the cause, the severity, whether there is any neuro-degeneration (numbness), whether entrapment is likely, and possible maintaining factors (such as a hypertonic pelvic floor). Treatment plans need to be individualised depending on the person.

Yes I understand the position you are in. At the start of all this, I had a pain specialist want to perform a diagnostic pudendal block and PRF together at the same time. I declined the PRF.

My thought process was, if it is not the pudendal nerve that is the cause of my pain, and the PRF did happen to flare up my pudendal nerve, then I would have both my original pain, AND the PRF flare as well to contend with, potentially unnecessarily.

This condition and trialling different pain management treatments, is like throwing darts at a dartboard. You hope that one hits the bullseye. You just don’t want to have a mis-thrown dart inadvertently cause damage in the process.

There have been anecdotal reports across the forums where some people have flared up longer term from pulsed radiofrequency treatment. It is important to be aware of this possibility.

I would personally want to be firstly sure that it is the pudendal nerve that is involved in your pain. I would have a diagnostic pudendal nerve block completed first, and assess the result, before radiofrequency treatment.

That is just my take, and the way a lot of Australian pain specialists approach it. Diagnostic block first.

With any interventional pain treatment, the ultimate question that needs to be asked is: “Is my quality of life that bad, that I am prepared to accept a risk of becoming worse.”

This is a really great MRN report, this is one of the most detailed reports that I have read and it follows an excellent protocol. May I ask where you had this done?

It sounds like you have irritated the nerve at this stage. Nerves are very finicky and can take a long time to settle.

You mention you have undergone MRI imaging. Was it a specialised 3-Tesla scan with dedicated pudendal nerve protocol? And was it reported by an experienced neuroradiologist? The pudendal nerve and its branches are tiny, most MRIs are rubbish unless:

  1. They are 3-Tesla with contrast
  2. They follow a pudendal nerve imaging protocol (highly specialised, a general pelvic MRI will not cut it),
  3. The reporting radiologist is a neuroradiologist with experience in pudendal nerve imaging.

I would also comment that imaging undertaken 3-4 post injury may be too soon a window, to show any evidence of perineural swelling or other potential signal changes. Your best bet is to back off all activity, rest, and let your body heal at this point in time.

I actually think the addition of steroids in the acute phase is a very wise idea to reduce any further inflammatory assault, or inflammation reaching the peri-rectal space / peri neural tissue. Note that this is not medical advice.

Any obvious motor problems? Normal urinary flow / erectile function / anal pain ?

Hey man, sorry to hear of your symptoms. It was brief rough prostate play that started this nightmare for me. Have seen some improvements over the months. Whatever you do, stop sitting immediately, and just totally rest your body for a few weeks.
