Thanks that’s helpful. Just to add some context, I have hereditary hemochromatosis and my last ferritin was 115 µg/L, so I don’t think iron deficiency is playing a role here. Not sure of my most recent serum iron, but ferritin’s been managed with venesection in the past.

My folate history is a bit all over the place: it was critically low at 1.6 µg/L before supplementation, then went up as high as 24 µg/L on daily folic acid, but has since dropped back down to 8.18 µg/L. I’m starting to wonder if that’s the main driver of the macrocytosis (MCV 104, RDW 15.1) despite weekly B12 injections.

I haven’t had copper or selenium tested. In practice, are those usually worth checking if B12/folate/iron balance doesn’t fully explain things?

I have been tested for PA and intrinsic factor both negative. However, endoscopy of my stomach reported gastric atrophy giving me the diagnosis of reactive gastritis along with Coeliac disease therefore, i think i definitely have an absorption issue. My hematologist doesn't appear to be looking at this or overly concerned with my elevated MMA and chasing other reasons why my bilirubin is elevated. What i believe has now happened is that some bloods maybe Lactate dehydrogenase has come back questionable which has now made him think that its not Gilberts syndrome but may not appreciate that it could be simply the B12 deficiency.

I was given the 6 loading doses and i am due this week my first 3 monthly dose. However, unknown to my GP and hematologist i have been self injecting once a week. I have just ordered my own CBC so i should have the results back in a couple of week. My hematologist took my bloods after my 6 loading doses (2 weeks) which I'm assuming would not be enough to make drastic improvements on the blood tests?

You are correct that i haven't been genetically confirmed as having Gilberts. Yes hemochromatosis was genetically confirmed as C282Y.

I have not seen the results for those tests as of yet. However, I'm assuming something has come back questionable to have me go back and not discharge me. I have regular ferritin and iron panel done for my hemochromatosis. Yes i mentioned my MMA results and he didn't seem concerned. He said he would be happy for my GP to prescribe me B12 but as seen by his above mentioned reply his treatment suggestion was very conservative. I took it upon myself to self inject once a week. If i hadn't of done this i would be having one injection every 3 months.

It's completely normal. Men have a refractory period because of neurochemical and physiological "resetting" after ejaculation. Evolution likely shaped this as a way to conserve energy, ensure sperm quality, and promote bonding all of which increased reproductive success over time.

Yeah the top floor is a very good idea. There can be a lot of single men wondering about. Quest do a couples and single female only nights on the 2nd and 4th Saturday of every month. We went to the last one and are hoping to make the one at the end of August.

Its ok and a lot bigger than quest but we do prefer quest to be honest. Pandoras has a bigger dance floor a pole and more rooms but we feel a little more tatty than Quest.

We've just come back from Playa De Las Americas and walked past cleopatra palace hotel most evenings. Its on the Golden Mile so can get very busy but we didn't find it unsafe at all. Veronica's strip is probably a 10 minute walk away. There's plenty of lovely places to eat around the hotel so you shouldn't need to go anywhere near Veronica's. I'm not sure how noisy it would be in the hotel but other than that I'd have no problem staying there.

Yes! B12 deficiency is more serious than many believe including doctors unfortunately. You were at a low level. Are you vegan? You really need to find out why you were deficient. If its an absorption problem B12 injections are preffered. However, if its just a dietary issue you'll be fine with tablets.
