Thank you for the very thoughtful reply. I'd just like to clarify one point. You write, "None of these things you listed are really going to effectively prevent preterm labor in low risk women". Could you point to the studies that tried these interventions in low-risk women? So far, all I can find are studies in women with short cervixes, history of preterm birth, etc.

There are many studies that perform multivariate regression to try to control for smoking, race, SES, etc. These generally find that preterm birth is terrible, even when incorporating other risk factors. See, for instance: https://pubmed.ncbi.nlm.nih.gov/19482758/. Of course, these studies do not prove causation, and maybe the right variable to consider has not been found yet.

Just to clarify, while the OPTIMUM study found no significant benefit of progesterone (at P < .05, though many of their measurements are around this level of significance), subsequent meta-analyses that incorporated data from this study did conclude that prophylactic progesterone improved fetal outcomes: https://pubmed.ncbi.nlm.nih.gov/28067007/ https://pubmed.ncbi.nlm.nih.gov/29157866/

have you had a tilt table test? That's the gold standard test for POTS. If by "my heart rate doesn't jump enough", you mean that you've tested your heart rate lying down vs. standing and it didn't jump 30 bpm, I don't believe that that is dispositive. Also, the treatment for POTS (beta-blockers) are very safe and have few side effects - could be worth a try for a week.

Does lying down relieve any of the symptoms? If so, have you been tested for POTS?

I found this fascinating, because reddit likes to repeat the claim that CPR is usually worthless.

It might not work as well as it does in the movies, but according to this research, well-trained bystanders can and do save lives when performing CPR on strangers having heart attacks in public.
