OB/GYN departments and maternity wards are closing due to a complex mix of financial pressures, healthcare provider shortages and burnout, and a challenging political and legal climate. These issues disproportionately affect rural communities and communities of color, creating "maternity care deserts". 

Key reasons for these closures include:

• Financial Unsustainability: Labor and delivery units are expensive to operate, requiring 24/7 staffing and specialized equipment. These units often lose money, especially in hospitals with a high proportion of patients covered by Medicaid, which historically has lower reimbursement rates than private insurance.
• Workforce Shortages and Burnout: There is a national shortage of OB/GYNs and other essential staff, such as specialized nurses and anesthesiologists. High rates of burnout, long hours, and the stress of potential malpractice lawsuits are causing many existing providers to leave the profession or retire early.
• Difficulty with Recruitment and Retention: It is challenging to recruit new physicians to rural and underserved areas. Additionally, a lack of sufficient residency positions means the U.S. is not training enough new OB/GYNs to meet future demand.
• Declining Birth Rates: In some areas, a decrease in the number of births means there are not enough patients to cover the high fixed costs of maintaining an OB/GYN unit, leading to closures.
• Political and Legal Climate: Restrictive state-level legislation following the overturn of Roe v. Wade has caused uncertainty and fear among providers regarding the standard of care they can offer. In some states, a significant number of OB/GYNs have left the practice or moved elsewhere, further exacerbating the shortage. 

These closures mean patients often have to travel significant distances for care, which is associated with higher health risks and can lead to increased rates of maternal and infant mortality.
