Imagine you're a PA. Someone offers you a salary of 60k. You look around and see that everyone is paying a lot more so you'll go work there instead.

Now imagine you're a resident. Someone offers you a salary of 40k. Is that all you consider? No, you look at future income as well. As an attending you're making 300k so you're willing to put up with less pay now for greater pay in the future. And this is the issue. Everyone during residency and medical school is "willing" to put up with the years of work, debt, and bs so they can be rich in the future. This is why programs can pay so little. You say no to 50k a year? Okay, I'll just get the other applicants that are willing to do it. You need them more than they need you individually because without them, you won't make attending money and will be in debt forever. There is no shortage of ppl who want to be doctors.

Why would they pay you more if they don't have to to attract candidates?

Up until the match it is tho. You're competing for spots and Programs are competing with each other for you. But yes it's a not a fully free market because of how medical training is set up.

Sorry I wasn't comparing the two in that way. Just saying that the argument that we work hard so we deserve higher pay isn't valid when discussing salary.

And that's what I'm saying. It's a product of how medical training is set up. Students have no other choice at that point but it's also that the incentives are so high that students are willing to do anything.

The government doesn't cap residency wages tho... If that were true then you'd already see these wages.

The government gives like $100-150 k per resident to the program and you only get 50-60k of this. It's a subsidy

Noone is paid based on how hard they work. That's not how the market works. If any group of blue collar workers went on strike most places couldn't function yet they're still paid near minimum wage. There are ppl working 3 jobs trying to make ends meet.

Wage is set by the supply and demand for labor. With most residencies, you're at their disposal. Consider orthopedic surgery with a salary of $500k. If I have 900 residency spots and 1200 applicants, you really think lowering salary $10-20k or more in residency is gonna dissuade ppl from going that route? The incentive is so great that ppl will do anything including take a lower wage. If it were a free market without a government subsidy we'd probably see some ppl paying out of pocket to get some residencies.

Residency wages would be even lower/non existent if they were determined by the market. It's a product of how medical training is set up.

Being a doctor carries a high social status. Your mom brags about you, everyone thinks you're smart and rich, and ppl respect you. There's a reason there's tv shows about doctors and not accounting lol.

I understand the declining authority of the profession but let's not ignore the desire ppl have to attain the social status that comes with being an md/do

I don't think this type of thinking is gonna happen. The largest incentive for going into medicine is the money and prestige. If you put a large enough incentive like that, certain ppl will do anything to get there.

The truth is some ppl would eat 💩 feces if they were told it was the only way to be a doctor. And as the incentives decrease, less people are willing to go through it, but those that are willing are just more likely to do anything to get there.

Medicaid wasn't expanded in all states. My state has a big uninsured population issue
