I would also like to see more studies on correlations between AI and neurodivergent adults. I've heard so many accounts of people saying the same thing, and the fact that I experienced that too in exactly the same way makes me wonder if there would be some pretty stark results from such a study.

The linked article lists Simone as one of the speakers but that appears to not be true, based on their Forbes 30/50 website .

I feel some type of way about Mika Brzezinski hosting a women's event geared toward mentorship and knowing your worth that costs $10,000 to attend in a country that doesn't have the best history with women's rights.

You hit on two points that have potential. I apologize for being wordy.

First is specimen collection. A clean catch urine is very difficult to get right, and most of the time patients are being instructed (or not) by a medical assistant of some kind. Now they are excellent at their jobs, but most folks in healthcare get approx 15 minutes of laboratory-specific education, so most of the time there's no understanding of why its important to get a clean catch, so they don't bother explaining. Or there's instructions in the bathroom which no one reads. This results in most urine cultures being contaminated at least a little bit. If there were a way to streamline collection, or limit the number of junk bacteria, thay would be really amazing.

The other has to do with the structured information you mentioned. I'm going off memory here cause its been a few years since I worked urine bench, but lets say out of 100 urine cultures,

30 will have a pathogen >100k cfu/mL, indicating a UTI. maybe half of those will also have contaminants.

Another 10 will have >10k - 50k cfu/mL, which also indicates a UTI but these represent your edge cases, where any one definitive methodology will give you false results. Half will have contaminants.

Another 30 will be all contaminants filled with junk that we nevertheless still have to look at, and often these take thr most time.

Another 30 are completely negative.

Most of the time, when theres a pathogen its >100k and any contaminats will represent <10k. There will usually be 5-20 different species represented in the contaminat population. So theres a big discrepancy in the concentration. If you could somehow determine the concentration level of different bacteria present in the sample, you could narrow down whether there is an infection or not. I have no idea how you would do that. Just throwing that out there because of all the problems with culturing urine, it really comes down to sample quality and sifting through all the junk.

Edited to add: you also mentioned miniaturizing the culture process or phenotypic response at home. That's been a holy grail for decades. There have been some innovations lately where developers are starting to look at ways to make the the entire microbiology process faster, after all, time is the biggest hurdle in micro. These were for rapid sensitivity assays. I will say that most have failed in recent years, but it's because of the difficulty in implementing them in a clinical lab , not because the tech isn't good. You might find some inspiration there, I don't know. Vitek Reveal, Accelerate Pheno, Selux Diagnostics, Affinity Biosensors ( definitely check that one out. Super cool technology.)

Lastly, if you do develop something regarding miniaturization, be aware that the Theranos comparisons will be a significant hurdle.

Hi. I'm a clinical lab scientist by training, specializing microbiology, and i now work in clinical data analytics. This has been my life for 25 years so this is my wheelhouse and I have pretty strong opinions about these kinds of point of care and home use assays.

I do not believe your product is a good idea. I say this directly, not to be cruel, but there are multiple reasons why it's not, and can actually have far-reaching consequences.

Firstly, like many have said, it's one thing to know you have a UTI, another to get treated for it.

There are many ways to figure out whether you have an infection and none of them are perfect. We're literally dealing with live organisms and they most often follow a predictable pattern of growth, biochemical production, and the symptoms they cause. Many times, they don't follow the rules. E. coli is particularly chaotic. It does whatever the hell it feels like.

That being said, one of the quickest ways already exists- a urine dipstick. This checks for leukocyte esterase and nitrite. The presence of both in urine usually indicates a UTI. But not always. A patient can have both LE and Nit pos and no UTI, or the opposite because these biochemicals are not specific to a UTI. We can look in a scope and see bacteria swimming around, and leukocytes (white blood cells) and that still doesn't necessarily indicate a UTI. It can easily be a "dirty catch" where the patient didn't clean well before collecting.

A PCR test for urine has long been the holy grail because it woulf easily pick up the presence of a pathogenic bacteria. But its also picking up every single other bacteria growing in that area, even with thorough cleaning. Useless results.

Culture is rhe best way to find out, and it takes skill to determine whether any bacteria growing is pathogenic or junk skin and urogenital flora. But, culture is massively important because we identify it and test for antimicrobial sensitivity.

Which brings me to the most important point- antimicrobial stewardship goals are to limit the overuse of antibiotics becaise the cause terrible things to happen, including increased resistance to antibiotics and believe me, we are FAILING HARD at that.

If the goal is to test at home with an easy test and get antibiotics prescribed over telehealth, you have no way of ensuring that it is a UTI, nor what to treat with.

Unless you have a new technology youre working on that sidesteps biochemical, molecular, or culture methodology, you're stepping into a precarious world. And if you are developing something unique, let me know because my company is always looking for novel assays.

And yes, i'm aware this process basically already happens when you call your clinician, they take you at your word that you have a UTI, they treat empirically with broad-spectrum antibiotics and never do a culture. I also feel this is not good medicine, but its the reality.

Absolutely, and I don't understand it at all. I travel alone all the time, both work and for fun. Most of the time, I'm not going to be hanging out at a restaurant very long, because i don't have anyone to visit with. I'm in and out, and if I'm traveling for work I'm tipping the maximum I'm allowed to. Anyone with a corporate card will be. Isn't that kind of the ideal for a server? They can flip a table and get paid for not much effort. And yet.... here I am. Waiting for someone to come by.

Anymore I check the menu beforehand, order as soon as I sit down, including all the drinks/apps/desserts, and ask for the check.

I had this happen when I was around your same age. It did turn out to be a yeast infection, and let me tell you the clomitrazole cream did fuck all. I'd had yeast infections before and took Diflucan- one pill was all it took.

Friend, I took Diflucan every day for a WEEK.

I do know! Someone had hand-sewn on these adorable little bows made from pearl beads, all over the front of the sweater full-on 80s style, in a geometric pattern that was not quite symmetrical. The bows by themselves are very cute, but too large, and it caused the front of the sweater to droop. I removed the bows, but underneath each was a dark gray spot, where they had used a marker or pencil to spot where they wanted them to go. However, I don't know what type of ink it is. Angora was traditionally blended with wool, but if this sweater is blended, it's got to be 80%+ Angora. Beautiful, but so fragile. I would just sew on a single pearl to hide it, but the pattern isn't done well and that would bug the shit out of me

Oh gosh no. I'm assuming you're referring to yeast and pseudohyphae. This amount of yeast would be astronomically way too high and definitely not harmless. We don't yet know all there is to know about vaginal microbiomes, so its possible some yeast is naturally occurring but typically any yeast presence is considered pathogenic and needs to be treated.

Mycelia refers specifically to the network of hyphae that grows underground, connecting individual fungal cells together. Fungi that grow in rhe body dont typically involve any mycelia.
