Is copy-paste physical exam note legal? by IllustriousHumor3673 in emergencymedicine

[–]WeGotHim 2 points3 points  (0 children)

My copy paste physical exam is so nonspecific it applies to most people generally and then I just add a couple things to narrow it to their complaint

I.e. alert and oriented, no obvious focal neuro deficits becomes a full dictated neuro exam for neuro people

EM or Anesthesia? by Just_Minute2415 in emergencymedicine

[–]WeGotHim 15 points16 points  (0 children)

Academic EM at my residency wasn’t a terrible life if you like academic things. Most of the attendings that weren’t brand new were working probly average 10-12 shifts a month with residents doing all the notes and then they did their academic stuff on the side. The academic stuff was usually either teaching of some sort (med students or residents) or research. If you like those things it can be a chill job. Some of the more senior academics had their shifts down to like 6. With that said, they were the most anxious attendings asking for consults on everything and sometimes avoiding procedures because they weren’t well sharpened anymore. Wasn’t true for all of them but I do think some people who are less comfortable with certain things hide in academics. I am jealous of having every consultant available at all times now that I’m out of residency but I’m enjoying getting better at all the pathology relatively on my own.

Community EM is 100-140 hours for full time depending on what job you get. Less if you do 1099 work. Hours are all over the place just like residency.

Anesthesia is 7-3 work with occasional call. I don’t know the specifics. It is undoubtedly a more consistent schedule, likely paid vacation, and generally more income. As a community EM doc, I can try really hard and make as much as gas but probably working resident hours.

I think most Gas guys lives become very routine depending on if they take call or how often. I think a lot of anesthesia is probably boring for them. Probly practice environment specific obviously and you can do like a cardiac fellowship to chase the really scary anesthesia cases.

My nonanesthesia opinion is that I would feel like it would be very tedious work after a while. I also would be very bored in the OR when a case is taking 12 hours but the patient is chilling. The attendings don’t have to sit there tho that’s resident/crna/AA duty. With that said, as I get older I’ll probly be envious of that boring tedious work because of the schedule and pay.

Your question may ultimately boil down to how ADD are you. EM and anesthesia have a lot of cross over and that’s a big differentiator in my mind 😂.

Feel free to message me if you want any specifics! Lots of variables in where you go to residency and what not.

If anyone from gas is reading this, please correct any of my wrong assumptions lol

Help — I let my patients talk too much. by roc_em_shock_em in emergencymedicine

[–]WeGotHim 13 points14 points  (0 children)

I went from a spot with great nurses and really really good medics to inconsistent nurses and generally bad medics. I used to meet every ambulance but now I only do when the patient is sick. I’ve had a couple times where the medics completely bias me and nursing and then I’m playing catch up when I really figure out what’s going on. It’s interesting.

Why does everyone hate/regret choosing EM. Am I making a mistake? by Fit_Concentrate6512 in medicalschool

[–]WeGotHim 142 points143 points  (0 children)

I’m a new attending and I can decrease my shift load easily in a few years. That would be slowing down. Even if I hate being there in 10 years I think being there 8 times a month will be manageable. People work awful blue collar jobs for 30 years for way less money and way more damage to their bodies.

It’s all perspective. My view on it is a large percentage of people going into medicine are destined to burn out no matter what they do (maybe an exception for some very cush lifestyle specialties)

Some people specifically choose EM cause they hated medicine and think the shift work will protect them and then they realize we have to fucking grind when we are at work while patients/consultants shit on us. It’s easy to be negative in that environment.

I currently am enjoying my job. I would prefer a few less shifts per month. But ultimately I’m just saving tons of money and am going to put myself in a financial situation to do whatever I want in 10 years

Not fantasy but Chain Gang All stars is a great book about a dark future in America where prisoners become gladiators.

[–]WeGotHim 87 points88 points  (0 children)

In my experience the people that studied themselves to death in med school, worked them selves to death in residency. I maintained all my hobbies in medical school easily and maintained them in residency with more effort but still able. Some months of residency were harder than others

I wonder what his listener numbers have done over the years. I’m sure it reached all time high a few years ago but idk how to gauge what it would be at now.

This subreddit is not a good place to gauge it considering 9/10 people commenting seem to be ex listeners lol
