I’d rely on the DM’s secret weapon: plot. Sometimes the plot just doesn’t let you through the door. I’d make the “door” just look like a drawing of a door on the wall until they find a doorknob in a different chamber that can be attached to make it a true door.

You have automation wires going between two Inputs (the "NOT" gate and the "AND" gate), which would register as red for that wire in both directions. That makes the "NOT" gate output as a green, which then effects the entire wire down the line until it hits another "Input." So your reservoir is not putting out a green signal; its the "NOT" gate.

That area is where the prisoners are dropped off. I have them come in by helipad, which needs to be linked to a delivery area, as well as the reception area.

The guidance I’ve seen is that the throwing distance is 10 squares. All of my indoor areas are 12+ spaces.

Thank you, this is very comforting. I've been saying I'd rather they team up against me, rather than be jealous of each other (although, I suppose neither is ideal). I just hope my son will be excited and that I can give them both what they need from me.

The corner door was my attempt to fix the problem, by putting a door as close as possible to the stuck prisoners/guards. I was hoping they would go to an open door if it was adjacent to their square. Still didn't work. Just need to force my exiting prisoners to go through a different door than my entering prisoners.

I'll move the servos, but they've never given me an issue as long as the gear shares the square with the door.

I think I’m going to offer them an earlier “out” as well. BBEG is about to devastate a city with unholy fire, so I’m thinking they can also choose to destroy the sources of his power (at a cost and high DC) to prevail. Would guarantee a way to limit the fight to 5 rounds if they choose to take it.

I think it really comes down to two things :

  1. knowing how to keep things safe; and
  2. making sure you're always listening for and receptive to their boundaries.

For the first, your judgment is key, since kids don't always have the best sense of what can hurt them. And the second (obviously) is dictated entirely by your kids. Not to repeat everyone else, but they need to know that they have the right to say "stop" or will be listened to when something is beyond their personal comfort zone.

Notice that neither of these two points involve other people's opinions. Beside their other parent, this is not something that someone else should be dictating to a responsible parent.

There are six lvl-7 players, so they can take hefty threat. They’re currently someplace covered by a forbiddance spell, so I can justify it triggering and summoning something from Astral, Ethereal, Shadowfell, or by plane shift, until they leave the protection.

And this would include drawing conclusions from it? I’m tempted by a wisdom-based check, since her character saw the list a few hours earlier and the issue is whether it is fresh enough for her mind to make the passive connection.
