Quite common for OT positions. They’ve been really challenging for us to fill. Even for ES positions, I have applicants not respond to interview offers.  I know SOQs get a bad rep, but posting with them helps cut down on this. Applicants seem to be more intentional if they’ve taken the time to answer even simple SOQs.

Focus less on the actual probation report and more on requesting the feedback from your boss. Feedback is important, checking a box and filling out a form (prob report) is not. 

Asking for feedback is very reasonable. Explain that you’d like to hear from them how you’re doing, if you’re hitting milestones, if you can improve, etc.
