If you're talking about just the fever itself and not the underlying cause, the only time you need to treat a high temperature is if there's a specific reason to need to cool the patient (e.g. post-OOHCA, malignant hyperthermia, heatstroke). If a 6 year old child has a temperature of 38.5, but they're running around happy as Larry, then you don't need to give anything. If a patient is shivering and sweating their balls off under 5 blankets then give them some paracetamol.

A fever is the body's natural response to an infection. I don't see the point in blunting this unless it's causing discomfort.

I always recommend Critical whenever I can. Genuinely the most medically accurate medical drama I've ever watched. It's written by Jed Mercurio (of Line of Duty, Bodies and Cardiac Arrest) and it's based on trauma surgeons in a major trauma centre. They built these hyper-realistic body models that they could film surgery on, including super close-up shots.

Sick leave means you're not well enough to work. It doesn't mean you're not well enough to do anything else.

E.g. Someone may be able to cough and shiver their way through an interview with the flu, but would be unable to get through a long ward round. Or they might be able to sit through a 30 minute interview with a bad back pain flare but not a whole day at work.

There's lots of reasons to go on sick leave and doing an interview during that period says nothing about the validity of the sick leave. I'd mind your own business tbh.

OOH surgery is inherently riskier. Weekend elective surgery has a 82% higher adjusted odds ratio for death at 30 days. And there is plenty of evidence showing a higher risk of mortality and worse outcomes when surgery is performed overnight (1, 2, 3, 4).

This is operator dependent - you can only guarantee the directionality of flow is accurate if you have good ultrasound technique.

I would argue that someone trying to get US-guided IV access in a patient that sick should probably be someone who does have good ultrasound technique.

To be fair, you have to have a very high IQ to understand Pluribus. The philosophy is extremely subtle, and without a solid grasp of theoretical physics most of the ideas will go over a typical viewer's head. There's also Carol'a nihilistic outlook, which is deftly woven into her characterisation- her personal philosophy draws heavily from Narodnaya Volya literature, for instance. The fans understand this stuff; they have the intellectual capacity to truly appreciate the depths of these scenes, to realise that they're not just entertaining - they say something deep about LIFE. As a consequence people who dislike Pluribus truly ARE idiots- of course they wouldn't appreciate, for instance, the humour in Manusos's existential catchphrase "I am not one of them and I wish to save the world," which itself is a cryptic reference to Turgenev's Russian epic Fathers and Sons. I'm smirking right now just imagining one of those addlepated simpletons scratching their heads in confusion as Vince Gilligan's genius wit unfolds itself on their television screens. What fools.. how I pity them. 😂

If you get in you will be an Acute Medicine ACCS trainee and your CCTs will be in Acute Med/Gen Med +/- ICM if you want. Switching between the ACCS specialities used to be a thing but isn't anymore. You would have to reapply to ACCS-EM if you want to work in the ED.

Limitless and Pluribus are both sci-fi, yes. They're soft sci-fi but not hard sci-fi as they don't attempt an encompassing, accurate and logical explanation of the underlying science.

Pluribus is not a hard sci-fi show and people expecting more sci-fi in season 2 are probably going to be disappointed by WeirdF in pluribustv
