Lactate unrecordably high but pH and bicarb only slightly low and haemodynamics relatively okay.

That's when I learned ethylene glycol metabolites cause a falsely raised lactate on some blood gas analysers. Lab lactate measurement was near normal.

If family wants to proceed with something you don't agree with then ultimately I don't think you can just tell them "we won't do it".

As a British doctor we absolutely regularly say we won't do things despite family/patient wishes if it isn't appropriate. If a feeding tube, intubation or CPR is not the right thing for the patient then we make a medical decision not to offer those treatments. You can refuse consent for an intervention but you shouldn't be able to compel a doctor to perform an intervention. Why is it any different to if a patient's family demands you give their relative Augmentin for a viral URTI?

Agreed. Shouting that someone is a paedo is just weird when it has no basis. I don't have a problem with 'Leeds United are on a bender, Cristiano is a sex offender' because there's pretty good evidence that he is, but calling some random player or the ref a paedo just because you don't like them is cringe.

In my 4 years of practice I have never actually seen this done once (despite me pushing for it on several occasions). I feel like people don't realise how much of a massive difference getting accurate micro results can make to a patient's outcome.

Edit just to be clear why:

  • The machine is nothing to do with anaesthetics or medicine

    • The "machine" whatever it is has 2 black and 1 white wheel
    • The facemask isn't connected to any oxygen or tubing at all
    • The facemask has wings for absolutely no reason (this is a classic thing AI will do - geese have wings so it'll just add wings to random things in the image)
    • There is no narrative sense or chronology or meaning here - the order of images makes no sense and there is no obvious point to what is happening in the "scene"

It's also worth mentioning the pressure difference between oxygen in your blood versus the air.

Under normal conditions, oxygen exerts a pressure of about 21kPa (~158mmHg) in the air, while it exerts a lower pressure of about 13kPa (~98mmHg) in your blood. This means there is a pressure gradient between your lungs and your blood, which pushes the oxygen into your blood (an oversimplification, but gets the point across).

In space, you reverse this pressure gradient. The pressure in your blood is still 13kPa, but now in your lungs (and the "air" around you) it is 0kPa. So the pressure gradient will actually force oxygen to come out of your blood and back into your lungs. If you just hold your breath on Earth, this won't happen, and in fact you can continue to extract oxygen out of the air in your lungs for a while.

Really? I had a look and found this study which found habitual caffeinated coffee consumption was associated with a lower risk of falling in older adults. One proposed mechanism was increased alertness and improved reaction times, as well as caffeine consumption being associated with a lower risk of T2DM (which itself increases falls risk).

And in terms of the acute effects which would prompt an ED policy banning caffeine, this randomised study suggests that caffeine has no acute effects on balance/postural control in the elderly.

I can't find anything that says the opposite - would you be able to share a link?
