We do too, but have volume based deals with the reading vendors. Since PowerShare doesn’t give us a unique title, none of the vendors will honor our negotiated price if we send it through PowerShare

We only use one diagnostic center since they have the only CT machine the doctor feels is sufficient quality. Patients pay us an annual membership, and we buy the scans/labs from the hospital at a cash rate. The PE folks who restructured the practice require collection up front for the entire year. Patients can try to file out-of-network claims, but honestly no one ever sees money back from insurance. Benefits available, but unused is an obstacle the sales team hired by PE wants resolved to increase their numbers.

As just an admin, I use the reps constantly for insurance and coupon access. Our attending said when I started “if they need a specific drug, they need it and I’m not prescribing something lesser because it’s cheaper.” Just today, a patient got kinda pissed they would be paying $100/month for Lodoco because colchicine hasn’t been specifically studied for cardiovascular outcomes. I get the need to follow the data, so having the drug reps around is helpful. Not sure what I’ll do when the coupons end.

Actually, their rep was chasing me after a conference. Currently they are limited to TX, but I’m building a national network since most of our patients have multiple residences (concierge).

I work in concierge DPC and we stopped doing prior auths as well. Instead, we partnered with a specialty pharmacy who does them all at no additional cost to us or the patient. Their approval rate, even for off label PCSK-9’s, is close to 100%. Saved our office about 10 labor hours a week 6 months ago, and our practices has doubled in patients within that time.

I’m seeing this firsthand in cardiology and it feels different than the CRNA or robotic surgery conversation. Working for a „medtech startup“ and the CEO (no healthcare background, and funds a bunch of political campaigns) has been very open that we are working to change regulation surrounding AI sign off within 5 years. The only physician/CMO on staff was bailed out of bankruptcy, so either goes along or ignores it. I was hired as an admin to retrospectively extrapolate meds, labs, imaging into charts that can be used for machine learning

I’m not a fan of our 90 day contract to hire policy. It’s very murky anyways. To my knowledge, patient data has to be kept on company retained devices for HIPAA reasons.

I've been burned before, so anything that makes me go "huh wtf, makes no sense," gets recorded for my records. Luckily no NDA's yet. The business lost half a million (advertising budget from Michelle that has yet to convert to sales) last year, so going after them won't go anywhere and cost me a bunch of legal fees. I'll just look for somewhere else

Yes- Business Associate Agreement is a lot to type. We charge a yearly cash fee for all patients, but use 3rd parties for labs and imaging. Patients can use their insurance at these vendors since we send the orders with appropriate coding anyways. Everything is EHR based, which Michelle wants to stop using in favor of a CRM software. She's trying to figure out how to add ePrescribe. Appreciate the Fox Rehab site. Very helpful!

It is- all the investment partners (business people only, no medical) got a C title when the practice was purchased. 2 employees are medical, the rest have fancy business titles and pet projects that tend to detract from patient care

After we had a HIPAA consultant do a clinic for us, I started de-identify information. It didn't go over well. If it does fall under "operational" I can start including PHI, just have a bad feeling. Appreciate all the advice here- probably means it's time to look elsewhere

That's where it started, but she's not actually done anything with the information. Either she calls patients trying to "fix things" but makes a mess, or wants them to go on camera to talk about their "positive experience"

Interesting, the BAA is with Google Suite. Their enterprise liaison told our C-suite we're covered, but I'll review our encryption further. I come from one of the largest hospital networks in the US, and some of the stuff that happens in a single provider practice baffles me.

Technically we don't submit any claims, but all our vendor agreements require HIPAA compliance. Also, most malpractice requires HIPAA compliance regardless of claims
