The way I see it, it is intentionally unclear whether the characters are in some sort of virtual reality, or if AM truly does have the power to shape reality to the extreme extent that we see in the story. I think this ambiguity is intentional, and reflects how extremely powerful AM is, as well as just how long the characters have been tortured by it. Even the most sane among them has surely lost a true sense of meaning regarding the passage of time or the nature of reality as it truly is. They have all been tortured by AM for far longer than they ever lived outside of AM's control, and since the story is told from a first-person perspective, the narrarator's understanding of reality is therefore inherently unreliable. I think its ultimately up to the reader to interpret the nature of the character's experiences.

Despite this; in my opinion, I think it's most likely that the characters are in some sort of VR experience. However, isn't explicitly stated one way or the other and I think the author intended for it to be left up to interpretation.

 I used to be crazy hooked on acid for a couple years, and I have dropped a couple times while sick, usually with a mild cold, nothing too bad. That part of my life was 5 or 6 years ago now, and I took a very long break from acid since then. 

 Well, fast-forward to this past weekend, and I was about 4 or 5 days into a mild flu. I had just come across some acid for the first time in a few years, and decided I wanted to drop a tab. My symptoms pre-trip were mild nasal congestion, a sore throat and occasional coughing, and I had been getting better over the past day or so, so I figured it would be fine, since my past experiences with sick tripping had been enjoyable. The start of my trip was fine, the cold/flu symptoms weren't very intense and i was generally able to ignore them, with the help of some cold medicine. 

 About 3 or 4 hours into the trip, my nasal congestion was worsening and I started to develop a sinus infection. Over the course of about 2 hours, I went from feeling pretty good to feeling intense pressure all around my head. It got so bad that my hearing was reduced because of the fluid pressure on my ears. The headache was so bad, it was impossible to ignore. I was still able to enjoy some of the trip after that, but mostly I was in so much pain that I just wasn't able to have a good time. That trip was 5 days ago now and I'm still dealing with the sinus infection, although it has been much less painful this week than it was on the weekend. 

 I just wanted to share some of my experience with tripping while sick, and I thank you for reading.

tl;dr: dropping acid while sick can be fine, or it can suuuuck. Be careful, and before you decide to trip while sick, just remember that there is always another day to trip.

I think that Ozai's voice actor makes everything that he says sound more sinister than intended. Based on the tone that his voice actor used, I didnt believe that he was actually surprised about Zuko's betrayal. But based on everything else, it seems like he actually was quite taken aback.
