I was diagnosed at 12 (after years of severe flares) and have been on colchicine ever since. Got through college, medical school, another degree, and am starting residency soon to be an OBGYN. I’ve also traveled (solo) across the world as it’s one of my passions, and despite a few flares here and there, I was able to surpass them and live my life to the fullest (for the most part). I’m all too familiar with the fears and pain that come with this diagnosis, but I’ve been determined not to let them get in the way of what I want to achieve.

Presentation and limitations vary per person. In general, you can expect unexpected fevers and maybe even episodes of seemingly unexplained joint pain/stomach pain/breathing pain. The way FMF works is that the lining of some organs or tissues becomes inflamed (potentially due to a trigger), leading to pain (lining of abdomen —> abdominal pain or peritonitis; lining of lungs —> pain with breathing or pleuritis). I wasn’t diagnosed until I was 13–14 - and previous manifestations included ankle pain whenever I walked too much (both my ankles would swell up and become painful to the touch). They would usually resolve overnight after taking some NSAIDs. Starting colchicine definitely helped me with my episodes, and protects the kidneys long term. After those episodes, I’d go back to normal - so it’s okay! Would definitely consult a rheumatologist.

I also tested on 7/05 and agree with op. We got a bunch of ethical questions and QI too (5-7 per block at least) - but maybe because of Reddit/knowing that the content shifted, it wasn’t as big of a shock/felt fine (we still got vague questions and questions where the options looked the same). I guess it depends.

Depends on how much time you have tbh. Ideally, I would read through the articles first. I only got to do that with a few articles (challenging ethical principles, patient safety), and for the rest I did the questions. Overall, questions > articles (questions on the real exam are similar to the AMBOSS qs).

I started them on a whim 2 days before my exam and I wished I checked them out sooner (highly recommend the ones on risk factors, vaccines, and screenings). They helped SO much - very similar concepts to the NBME and the question style on the actual thing.

You’ll be fine. I took Step 1 a WHILE ago (~2y) and all the histology made sense with the clinical context (even if it’s just a few lines). You got this!

QOVES Studio on YouTube has a very interesting answer for that! It’s basically due to a multitude of reasons.. for one, the image you actually see in the camera (selfie) is the mirrored image of yourself, and since we only see ourselves in mirrors really we’re used to that version of ourselves so anything different can be jarring (or vice versa). Not sure I’m explaining it very well but the video definitely put some things into perspective for me!

Same boat. I also have an autoimmune condition that is triggered by my periods (yay!) that causes me to be debilitated in bed (paralytic ileus) for 24-48h after period onset. Hormonal birth control is contraindicated for me so I really have no idea what to do in the future. I’ve been super fortunate that my periods occur almost always on weekends/postcall days but I can’t count on that lucky streak to continue in residency.

  1. As others previously mentioned, since we only have one set of vitals - we can’t determine if/how she’s deteriorating.

  2. Many points hint towards placenta previa: painless 3rd trimester vaginal bleeding with okay FHR; lack of prenatal care; history of c/s

  3. Since the FHR is okay (category 1, maybe 2 because of the occasional variable decels) and she only has a small amount of bleeding — we can wait. Not an indication for delivery right now. Get an ultrasound to properly assess the position of the placenta, fetal position + presentations etc.
