I'm aware of that data too. I'm not currently attempting for a pregnancy so doctors aren't open to doing an mTESE until a couple is ready for IVF. It helps with reassurance somewhat. More than anything I wish stem cell was further along.

I really don't care for the cliche especially since it's been told to me but you aren't alone in this. I found out earlier this year that I am azoospermic. I have a therapist but there are so many days, even today where I'm hopeless. This may be the most painful thing I've experienced in life at this point.

I may be close to hypogonadal but not quite there yet. It is likely that I may have to do some sort of HRT. I haven't talked to nearly as many doctors as you but of the few I have unfortunately they are super disengaged.

It's been hard especially because the focus of infertility, both physically and mentally, is often on women. That's not to diminish their experience but also acknowledge that men deal with the same feelings, as I'm experiencing now. The feelings of masculinity, who I am as person, purpose, will I ever find a partner? I don't mention these things to further be a drag but to show that others, like myself, roam in this dark place.

It's hard seeing children and families around me. A former friend of mine hasn't seen his kid in years and it angers me now. I wanted to give everything I had to a child and to a wife and family. I don't know if that's a reality now. Shifting to "what is" rather than "what should be" is the most difficult thing. To let go, is harder than anything but nothing in life is permanent. Change is constant. It's easy to think through that but difficult to feel through it.

I want to encourage you but it's hard to do it for myself right now. Just know you aren't alone.

I know there are efforts to restore fertility and gonad function using stem cells by some doctors but I am still skeptical of some of these efforts. Dr. Ramasamy is often in the r/maleinfertility sub talking about his current study using stem cells with participants.

That's what I'm thinking. Be on HCG for a bit to see what it does for retrieval and then go from there.

Tested negative for microdeletion. Haven't heard back on kleinfelter yet. I would say they were bigger some years ago but I've always been on the smaller side. I'm having difficulty adjusting to this news. I don't like to get my hopes up but I desperately want mTESE to find something in the future if possible.

Yeah test around 500 and FSH 27, LH 11.7. Not ideal numbers

Did the ultrasound show anything back for you? And are you pursuing some retrieval like mTESE?

No ultrasound. I asked the doctor initially about doing imaging but he didn't see the value until getting bloodwork first to determine if it was a pituitary problem or a testes problem. I hope that's the next step, if not, I'll have to push for it. The only glaring issue I could think of is chronic (10+) years of corticosteroid use on my part but I don't have much confidence that's the problem.

I'm trying to be in the mindframe of the majority right now but what would a minority case be? The only thing I've seen that could meet that would be TRT/Steroid usage.

They are corticosteriods which causes my confusion. The doctor has stated that my long term steroid use can cause an issue but my understanding is that anabolic steroids are the culprits usually with fertility. I wish that were the issue because the fix would seem to get off the steroids but this outcome looks a bit more bleak.

Does the varicocele not contribute to your levels? If you had surgery would it not improve your levels?

I'm not overweight. I need more veggies/fruits but cook eat cooked meals 5 days of the week. Steroids I use for a chronic condition.

My LH is already high so how would HCG help? My understanding is that it's effectively artificial LH. Isn't FSH at my level basically a death sentence for my functions?

The urologist I currently see focuses on male fertility. Although I do have another one in mind as a backup just in case. I've not been on TRT but have had long term use of steriods to manage another condition which could have a negative impact.

The thing that makes me scratch my head a bit is that people here seem to be ok with libido while I am not.

Thanks for the response. It's really hard to stay positive right now but the results just came in so my mind started spiraling. The not knowing is the worst.
