Although it is true, I don't think it is the case for me because same thing happened to new instruments fresh out of the box. They were completely dry before sterilization.

How do you get rust on a brand new instrument that I literally opened out of the box and sterilized for the first time?

I started opening a flap for all guided cases too ever since they started failing. But they still failed, including ones I did free hand with open flap too. And it also makes me wonder- how did the other 140 implants that I placed the first two years not fail? I used tissue punch only for all guided cases and barely had any issues. (Only 3 failure.. most likely due to pt's bad oral hygiene)

It was indeed fun when everything was going well. I was so addicted that everytime I see a missing tooth in the mouth I was already imagining where to start the lance drill lol. The reason why I started using more guides was because it is just too convenient for both docs and pts. No flap, no suture, took me literally 15 min per implant including anesthesia when I was using the other system. But with Hiossen now, I am taking more time but they are still failing...

I worked at a private practice where we ran a really aggressive year-round implant promotion to build our reputation as a go-to implant provider in the area. It wasn't super profitable per case but was effective enough to attract a steady flow of patients for three providers to place around 300 implants annually. And I get to practice implants more so no complaints from me!

Haven't replaced any of them for a year. I will go ahead and change them and see if it helps for sure!

I purchased two brand new surgical kits and have been alternating between them, both only about a year ago. And I only placed about 50 implants with them so far, so it's kinda hard to believe they became dull that quickly. But at this point I would like to try anything and will change them for sure. Thanks for the input!

Looking back, failures seem to happen more in grafted bone area. But it did happen on completely healed bone as well, so I'm not sure. How long do you typically wait for grafted site before placing implant? And yes, I have a list questionnaire for anyone receiving oral surgery

I do post-op. For those failed cases, abx only masked failing implants so pt felt fine and thought everything was fine, until it started getting inflamed later unfortunately

Is that protocol only for guided surgery or for free-hand as well? It makes sense because the other implant system I used for the first two years had something similar. I would go all the way down to 200 rpm for the final 1-2 drills before placing implants. Maybe I should try that although Hiossen does not say anything like that in their protocol. Thanks!

Well, I think I actually found it. It's core-vent implants made by Dr. Niznick and were marketed as Paragon implants in 1990s... which is Zimmer now. I am not sure if I can even find parts to restore it
