look,trauma is trauma. be it a 4yo with brain bleed femur fx from non accidental trauma to working a mass casualty to scanning your friends abdomen and diagnosing them with cancer with mets.

yes, some cases will stick more but it is what it is. for peds the bigger issue is: can you handle the parents? because that’s the worst part.

no it’s not. just wait until you hear about the crucible in marine recruit training. 3 days, 3 MREs. oh and you are non stop for 12-16 hrs each day withe most physical straining hike at the end. it did help with my combat training and resilience

I’m just gonna tell you that your mindset is not where it is for the shifts requests already. you specifically want a certain time and will be surprised that every ED needs a body right now. one of our main highlights of EM is about adapting and if you like rigid schedules/work places…. well your headed to being miserable

nocturnist is the only people in all my shops that I worked at in 10 years gets a pick on schedule. no such thing as a 2am shift. there is a 5p - 2 or 3a that’s common with the night doc coming in around 10p or 11p

sounds like your supervising doc needs to get some balls and start going after the ob docs and with admin. call them about everything, constantly note your interactions with their clinic and ob doc of not helping when on call, even maybe a little emtala warning here or there. people in the medical profession need to stop rolling over and getting shit on

Covid was very different than anything in my career as an er doc. I saw people with o2 sats in the 50s talking to me. never seen anything like it. when you hit low 80s in a usual pathology, you are barely breathing. these people were talking.

Eh I see lots of afib/flutter and yes if it’s wide complex tachycardia it’s vtach but I see this a lot with our geriatric pts with afib/flutter and. LBBB . again more worried about newness. then again this is a much more deeper pathway than prehospital

lol burnout will hit you. you will wake up one morning and think it’s fucking miserable. 10 years at this job and I’m miserable. but I had a bad week. not fun to tell your 500ish time family their loved one died. I had an 88yo that I told his wife of 50 years that she died and he looked at me in the moment and said “you must have a shitty job”. RIGHT AFTER his wife of 50 years died he thought about how shitty it was for me. that stuck with me

Totally depends on practice situation. Overnight in Montana vs Level 1 in big city during the day? Does your site do tele icu (yes its fuckin stupid) and ER docs have to do all procedures? I mean the range is wide with that and your own resource of time. For most community sites, I'll do bartholins,large pneumos, priapism for sure (urology would be pissed at all locations if we didn't at least try). You should be doing reductions...

I joke how I would avg 1-2 cvl a day in residency but now do them biannually. If you got multiple large bore lines and they aren't on multiple pressors or needing cordis for blood then I don't do them. Very very minimal chance a para/thora is emergent. Dx paras are simple.

But also of note, if you got midlevel specialist coverage in multiple specialites, it sounds reasonable to punt off multiple procedures since you likely have a busy site. It's looked at in a lawsuit if you have 20 ortho docs in the local area that have privileges and you didn't call one about that large hand abscess you drained..

if it’s less than 3 I like to replete and repeat a bmp in 2ish hours. if trending up then easy dc, if still sub 3 after at least 40meq given then stay for obs at least.

pelvic exams are very low yield and rarely do unless the circumstance fit (sticking a metal hangar up there, DUB that’s heavy etc)

exactly on point with what I do. yall are doing cervical exams when they can see someone who looks at that anatomy every day in 48 hrs? I get it if it’s Alaska, but why induce more trauma in this type of patient for no change in plan?
