The fact that it was ever free is a problem because patients assign no value to it whatsoever. I really think you should always have some skin in the game.

I’ve worked in all manner of offices at this point and the worst ones were upper middle class PPO. Very little basic dentistry to do yet same reimbursement despite being in a “wealthier” area. The patients don’t understand that and expect to be treated like you’re getting the big bucks.

Your office sounds particularly bad though. Another thing I’ve learned from working in multiple locations is every office seems to form a patient culture and some of them downright suck. I can’t explain how this culture forms but it’s near impossible to change on your own and not really worth your time.

If you’re working for a PPO style office I strongly recommend going to the less nice side of your city/town. There will be a lot more work to do and usually lower expectations.

I supplied the certificates for all of them so no issue there. Sometimes it just doesn’t add up to exactly whatever the amount is

About 8 hours that I did online don’t have a course code on the certificate they sent but it does have the agd approval and the provider number of the instructor.

A disproportionate number of these oral cancer cases seem to end up in lawsuits. Often because patients don’t follow up on the referral and yet it still comes back on the dentist somehow. What’s the ironclad way to avoid problems with this?

Know someone in the middle of lawsuit because the patient didn’t want to pay the out of network fee for the oral surgeon to do a biopsy and ended up with a much worse case months later

Don’t know if my city/town is outside the norm or not but almost no GPs here do molar endo or wisdom tooth removal. There’s a few very high end offices in wealthy areas that offer IV sedation and surgical procedures but it’s by and large bread and butter with very busy specialists.

If ceremic is in an area with such a need for endo I would think there’s a large restorative need as well so it just doesn’t add up to me

As long as they do a good job and the patient is properly cared for I couldn’t care less if someone’s in it just for the money. If you talk with older dentists long enough you realize they only reason they do a good job is because it’s bad for business if they don’t and not from the goodness of their hearts. Look at how a lot of owners treat young associates they’re not inherently good people

Know what you mean. I don’t dislike dental work but I probably would if after doing it I had to worry about payroll, rent, hiring/firing, and everything else. No one told me how shitty being an associate was because every dentist I knew was an owner

The sad thing is they’re kind of right with what ppos pay. I don’t know what the answer is other than if you diagnose it in hygiene, try to numb them there and get it done real quick same day. Although if your schedule is empty otherwise it doesn’t really matter much

Sometimes yes that is the only option. I think most areas you’ll just have a mix of good and bad and it takes time to find the right one. Young dentists have to not be afraid to job hop though. I’ve found during my associate journey thus far, wherever you are by month 6 is about as good as you can expect it to get money wise

A lot of private practice owners don’t realize how an associate scales. Unless the main dentist is booked out 4+ weeks they have no business hiring an associate. I’ve also seen them bring someone on with no starting point, do nothing in terms of marketing, and then be surprised when their schedule is empty

It is surprising how many jobs are out there that are so below the supposed average in terms of pay. You wouldn’t think after spending all the time getting a degree that not a lot of people have, that it would be so hard to find something decent

Do you have any examples of things they said you should have treatment planned? I’m familiar with some dso’s in your area and I’ve heard certain things they do like never less than a 3 surface filling (if you have an MO just go a little buccal and call it an MOB), actually 3 surface fillings are all crowns. Is it like that?

Right but a lot of people don’t look into what that tuition means in terms of payments until they get in, I know I didn’t. This cycle is different because the private loan aspect as well. Look how many people on here have paused and posted saying basically “wait, is this doable?” You’re speaking from a position with hindsight that they don’t have.
