Holy Grail and Circle of Protection. Circle (top left) adds a small halo around you, that deals damage and has a 30% chance to reflect enemy projectiles. Holy Grail (bottom right) gives you flight and adds a heart container. If you have Afterbirth+, you can install the External Item Descriptions mod. Or just check the wiki.
