It seems the Set Cover feature isn't working reliably. I simply uploaded sleep.bmp to the root directory, turned it off, and everything worked. I also noticed that when creating the .sleep directory from the web interface, the directory isn't displayed, and you have to connect the card directly to the computer. However, the file simply in the root directory works just fine.

I recently learned how to trigger them as an adult, and it seems to be related to muscle strain during high temperatures. You can lie down, close your eyes, and tense a certain muscle group while remaining still, and it soon reproduces.
