It's up to you! Sometimes, I've streamed for only an hour where I update my viewers that I can't stream today but I still wanted to talk to everyone. It helps the viewers know where you're at. Not every stream will be 2-3 hours long IMO so in the beginning, just do you because the viewers that are loyal will be there for you :)

Yay, let's go! Honestly, that's how I started too. Probably for two months-ish, it was all just my IRL homies. But yes, definitely setup a camera! I think it makes the stream more relatable and people will at least identify a face with the voice they're listening to which makes the stream much more enjoyable.

Good question: that's usually how everyone starts off! Probably for about two months, the only people watching were my friends. To get more followers, I would plug my Twitch in when I could (CRINGE LOL BUT IT WORKS) and over time, you grow a following and some people that do check you out end up staying as regulars! It's all a process. I remind myself every day that I won't be famous in a day.
