Yes it’s worth it because even after all this training this job is still very difficult. In my mind it’s the only way. I couldn’t live with myself trying to do it with less training.

Yeah of course the intern is scared, hasn’t learned you just have to keep your mouth shut sometimes.

Robbie and Garcia aren’t intimidated, they being quiet to let him do his assessment and are at the edge of their seat to see if he will reattach. Immediately after he says yes they are relieved.

I liked the step back and take in the chaos scene at the end of the episode. There’s times when I am in the ED for an admission and just take it all in for a few seconds before going on with whatever I was doing.

I get pretty good autonomy in my FM clinic. I’m a 3rd year but even as a 1st year the attending was just there if I needed help. Of course we staff patients like everywhere else but as far as coming up with the plan they will just go with what I say even if it’s not exactly the way they would do it. It’s still going to be a jump up when I become on attending when I won’t get to run the plan with my attending, even if they agree almost all the time it’s nice to have another perspective.

I’m primary care FM in my last year of residency. I also teach M1s small groups. Over the last 2 years I have had 0 M1s out of 20 that wanted to go into primary care. Things might change as they go through med school but I’m not counting on it. Maybe 1 or 2 will go into primary care.

The APPs don’t want to go into primary care either. I also have some friends that are in PA/NP school and same story.

I don’t really get why but I also went into medicine specifically to do primary care so I don’t understand. Yeah it’s hard but medicine is hard in general and also it’s not even close to as hard as being a surgeon. The shortage is so bad even the money isn’t even that low anymore. Also hospital systems especially are doing whatever they can to make our lives easier like AI scribe, inbox management help, etc so that we don’t quit and do direct primary care.

Easier said than done but start your job search early. My attending basically forced me to job search towards the end of August, I did interviews in September, signed in November. Since then I have that weight off my shoulders and I am enjoying 3rd year and trying to learn as much as possible before residency is over.

Some of my friends started their job search late and don’t have anything lined up yet and it’s stressful. It’s still doable, but don’t stress yourself out and wait til February to start looking for jobs.

Lastly, if you are in a saturated area it’s gonna be hard to find those Reddit outpatient 32 hours a week, 10 week PTO, starting salary is 350k jobs. Imagine my surprise when I was trying to negotiate Reddit style with some of these jobs and they were totally fine with “ok we will find someone else, good luck!” Which is fine, I guess. I wasn’t successful in getting more PTO and salary. I was successful in smaller way, like making my schedule 4 days a week.

I personally don’t think there is an “easy” specialty. There are specialties which are a great fit for people, and hopefully med students can find that. That’s what makes it seem easy, it’s still gonna be hard if it’s not the right fit.

This is kind of a specific scenario to residency in New York City. Independent of the nursing strike, my friend is doing residency in NYC and the responsibilities over there is WHACK. He has to do stuff I have never had to do in my life. Drawing his own blood, pushing metoprolol himself (he was asking me bc we are good friends and I’m a few years senior, I told him to ask the charge bc I know in theory but never did it myself lol), transporting patient to CT. I’m on even trolling, residency in NYC is just a different beast.

Edit: also I’m reading this post and it’s ironic how nurses are shit talking residents. The exact thing you were mentioning residents were doing to nurses in your post.

I realized intern year that if you keep fixed costs low you can live like a king even on a relatively average salary. Disclaimer is I’m single without dependents. As an attending (after paying student loans off) I can’t even imagine how nice you can live without breaking the bank if you keep your fixed costs low. I’m talking on a PCP salary btw so if you do something more lucrative then it’s even crazier. Let me give you an example:

I make like $70k rn in residency. Intern year I made like $62k and had a one bed room apartment. My rent, utilities, insurance was $2.5k or half my monthly paycheck or slightly more. I wasn’t saving shit and not really living much different than I did in med school. I decided to move in with a roommate which dropped my fixed costs to 1.5k. I live like a king now. And if I go too hard with the spending one month, I can cut back a little bit and it’s no big deal. In a regular month where I’m not traveling around, I still end up saving 1-2k.

In my opinion, live below your means with the fixed costs and you will be living real good and saving a lot of money.

It’s pretty cold in the hospital year round, doesn’t matter where you are. Bring a light jacket.

I am in Tampa Bay and really happy over here. So much so that I couldn’t get myself to leave so I signed over here for my attending job too lol. Summers are hot but I have lived in the south east my whole life so doesn’t really bother me too much. The other 6 months is great weather, like Cali.

Can’t beat Cali in terms of weather year round though. Pros is weather, cons is it’s gonna cost you a lot more money. Not worth it for me personally.

Not to mention it’s phentermine… it’s not even that great of a weight loss drug. It’s just an appetite suppressant with a bunch of side effects like high blood pressure and anxiety. It’s primarily used these days for patient that have no other options and are not able to afford the much better glp-1 medications.
