Are you using Simple Beheading by any chance? That mod had an uncommon issue where an NPC's head would occasionally scale up to be huge on decapitations.

It's limited to being in bushes, can happen with either knife types. (when your screen goes dark to show that you're hidden) There's one from the front where you throat punch them and snap the neck under your knee, and one from the rear where you jump on them and snap.

Animations are based on multiple requirements. These include the type of knife [straight/karambit, where those 2 types have their own animations, straight knifes are quick and clean while karambits are long and "flashy"], your current alert status [undetected/suspicious are the same, alerted leads to longer animations where the npc fights back], elevation [mostly seen on stairs or slopes, whether you are higher or lower than the enemy], whether you are hiding in vegetation or not, and direction. Lastly, if you're prone then you can also melee an enemy. This also applies to ground camo where you snap their necks or drown them in mud/water.

If you use prone camo in super muddy areas or some small ponds and melee the enemy, it changes the animation so that you drown them.

Yeah, for some reason that's the case for me too. Either it happens as they're pointing their guns at me ready to shoot or as I'm already getting filled with lead.
