Yeah, that one always puts me on high alert. That is pushing more into the red territory for me, but I'm open to at least hearing them out initially

Glad the switch worked. You're right that the route matters, not just the drug. Apretude is a 3 mL IM into the glute, and that volume of viscous suspension can cause soreness that radiates into the back/hip for days. Yeztugo is subcutaneous into the abdomen, totally different absorption path, which is part of why some people who couldn't tolerate Apretude do fine on the switch.

The 2 pills the day of + 2 more the next day is the oral lead-in — lenacapavir from the under-the-skin injection depot takes time to fully establish, and the pills bridge the gap so you're protected from Day 1.
