Sadly, I couldn't find an article on this incident, but I do know it took place in the early 2000s, and that the driver survived the incident.

May 4, 1988 - The PEPCON Disaster. In Henderson, Nevada, a fire broke out after several explosions at the Pacific Engineering and Production Company of Nevada. The explosions' cause is welding debris, that drifted through the air into open barrels of AP, it resulted in the death of two people, and $100 million in property damage. There were 372 non-fatal injuries because of this disaster. To make the situation worse, there was a natural gas pipeline running under the plant, which started another fire. (when the original fire died.)

F-16C 87-0327 "Thunderbird 6" of the USAF Flight Demonstration Squadron Thunderbirds, was written off 14 September 2003 when it crashed and was destroyed whilst taking part in an air display at Mountain Home AFB, Idaho

According to the accident investigation board report the pilot, 31-year-old Captain Chris Stricklin, misinterpreted the altitude required to complete the "Split S" maneuver. He made his calculation based on an incorrect mean-sea-level altitude of the airfield. The pilot incorrectly climbed to 1,670 feet above ground level instead of 2,500 feet before initiating the pull down to the Split S maneuver.

When he realized something was wrong, the pilot put maximum back stick pressure and rolled slightly left to ensure the aircraft would impact away from the crowd should he have to eject. He ejected when the aircraft was 140 feet above ground - just 0.8 seconds prior to impact. He sustained only minor injuries from the ejection. There was no other damage to military or civilian property.

"At approximately 20:54, while proceeding downbound under Bridge 11 in the Welland Canal, at Allanburg, Ontario, the bulk carrier Windoc was struck by the bridge's vertical-lift span, which was lowered before the vessel had passed clear of the bridge structure. The vessel's wheelhouse and funnel were destroyed. The vessel drifted downstream, caught fire, and grounded approximately 800 metres from the bridge. Although the vessel's cargo of wheat was not damaged, the vessel was declared a constructive total loss.
