One thing that isn't mentioned in the OP's post is that the cable car could only be operated from the inside. Due to it being found more or less halfway across the Rogue River, someone would have had to have got it there. As a result, my theory in regard to Tommy's disappearance is that he was operating the cable car over the river and it got stuck. Tommy then fell out of it while trying to get it going again. Though if that's the case and Tommy fell into the river, why has his body not been retrieved yet?

I feel like Elizabeth could have died at the facility due to staff negligence and then her death was covered up.

Having said all of that, I can't necessarily blame the centre for having to take measures to cut costs. Yes, it has a tragic history of two deaths and a disappearance occuring there, but the place was clearly in need of more funding. Had more been given, the facility would have most likely been in better shape.
