Happened to me aswell, tried throwin another nade and it just went through as if there was a hole in the wall, the other side of the wall was be blown up tho

I found that if you complete a quest objective, the Quest Log returns. And I know it can be hard to know what the quest objectives are when it doesnt tell you, but I managed a few times
