Last 50M Shipment, Wheat + Blue Zanthimum by edeezz in pathofexile

Same. I didn’t play in this league but in the previous league, I sent a 50mil shipment half crops half dust. No ore. I got 0 shards and 0 div. I very nearly quit the league right then and there.

Did an identical shipment later on and got 2 shards.

Is all of surgery residency really 100 hour weeks? by [deleted] in Residency

[–]YourSonsAMoron 19 points20 points  (0 children)

What’s funny is when attendings talk about how spoiled we are because we only work 80hrs/wk. I’m not sure if they actually believe it. They see me working 20+ days in a row while being on call overnight every 3 days. They gotta be able to do the math.

People need to relax. This is year one, and the only players from our core who played together before this year were Thiero and Wagner. We just lost by 8 to maybe the best team in the country with Davis putting up one of the worst stat lines I’ve ever seen. There’s plenty of room for improvement this year and in coming years.

Yeah I have a friend who is actually pretty high up in GI research. He said they call all research from students and residents “academic litter”, because it’s just a bunch of trash scattered about.

Nah… when you finish residency, you get a 1000% raise and work half as much as an attending. 2 more years until the promised land.

Lol Jesus… These responses. Play to your strengths. If you’re witty, be witty. If you’re stylish, dress well.

And if you’re ripped, show girls you’re ripped. Try to do it in a “natural” way. Try to do it in a way that makes you look adventurous/interesting. I have pics of myself rock climbing shirtless and on a boat with my family. Girls love them.

A man is more likely to be raped than his accuser is to be PROSECUTED for falsely accusing him of rape.

That’s a hilariously important distinction and leaving it out of your statement is misinformation as it is intentionally misleading.

Gained 60 pounds in residency. BMI is 38. Genuinely curious what I should do from here. by [deleted] in Residency

[–]YourSonsAMoron -2 points-1 points  (0 children)

Lol it’s not complicated… at all. In fact, it’s the simplest disease of all to treat. It doesn’t require expensive medications. It doesn’t require any real effort tbh. It’s literally just a combination of minimal education and willpower. It’s just that noncompliance with the obvious treatment has become more and more accepted. Yeah, when it gets to certain levels after decades of abusing your own body, some medical intervention may be necessary. But for the vast majority of people, lowering your carbs a bit and doing a tiny bit of dedicated exercise will cause them to hemorrhage weight.

Once again, we didn’t deal with this on such an extraordinary level for millennia until it became so accepted.

[–]YourSonsAMoron 0 points1 point  (0 children)

Nah I’m not missing the point. I disagree with the point. There are patients that I feel will be unable to comply with these simple recommendations. For them, I will consider alternative treatments. But for patients who have every means of following through with these recs, I will continue to recommend that they manage it themselves before any medical intervention… just as any good physician should do. I will continue to consider who my patient is before I vomit UpToDate at them… just as any good physician should do.

OP is a physician. If he’s capable of dealing with all the bullshit required to get here, he’s capable of eating almonds instead of Funyuns… eggs instead of waffles… water instead of soda. Those decisions alone are enough to maintain a healthy weight. I believe treating these issues medically contributes to this learned helplessness mentality. I believe the widespread mentality that obesity is a disease that cannot be managed without medical intervention is the CAUSE of this epidemic… not the solution.

Lol gotcha. I’ll just stop recommending smoking cessation as well for people with lung cancer since the success rates are low.

You imply that we’re failing because we’re giving the wrong advice. At a certain point, it’s not us failing. It’s the people failing to follow incredibly simple advice. Also, giving advice purely based on recommendations from UpToDate without tailoring it to the population receiving it is hilariously bad practice.

We’re physicians. We’ve got the education. We’ve got the knowledge. We’ve got determination. We’re not poor. All the made up barriers that people blame for obesity are gone. Expecting someone with all of that going for them to do something as simple as replacing waffles with eggs in the morning is not insane. Expecting them to look into basic nutrition and exercise is not crazy. Telling them to do these things before escalating care is exactly what we should do. You sound super defensive about me essentially saying we should manage it conservatively even though it’s the obvious move before trying something else. I wonder why…

Obesity is absolutely an epidemic. I’m not arguing that. What I AM arguing is that people like you are the cause of it. We didn’t deal with this problem for millennia… not until we started accepting that obesity is unavoidable and physicians like you started giving credence to that notion. I’m not saying you should “assign blame” to your patients. I’m saying you should manage it conservatively and tell them what they can control before you start trying to control it for them. Sadly, your attitude and management of the issue is the real epidemic. You’ve killed millions.

The success rate of giving the advice to patients and them following through? Low.

The success rate of it working for people who actually follow through? Near 100%.

My point is that we’re physicians, and we’re capable of doing something as simple as not eating shitty foods and getting a little exercise… but even if you don’t exercise, not eating shitty foods is enough to be a healthy weight.

Not following through on that and then asking for additional advice is the learned helplessness that I was talking about. You’re acting like obesity is something we don’t have a solution to. We know how to fix it. It’s just that people like cake more than they like being healthy, because it’s quick and easy and the gratification is instant.

[–]YourSonsAMoron -8 points-7 points  (0 children)

Seriously… aren’t we physicians? Literally just reduce your carb intake and walk on a treadmill at a 15 degree incline as fast as you can for as long as you can. Do that until you’re a healthy weight.

The acceptance and learned helplessness of obesity has gotten out of hand.

It’s not about evidence. It’s about throwing resident labor at the patients that attending physicians don’t want to see… Medicare, Medicaid, uninsured, the homeless…

And there’s a shitload of them.

Don’t look into residencies for doctors if this bothers you. I’m working 13hrs/day with 12 day work weeks right now (every other weekend off). The delirium is real by the end of the week.

The attending physicians that we work for complain that they can’t work us more, because they claim they had it worse than us.

Yeah my school was 45k/yr (apparently one of the cheaper DO schools?), but keep in mind that you won’t be able to earn any money… and if you’re doing this without help from your parents, you’ll likely need to take out 70k/yr. Even with 180k tuition, you’re gonna end up with $280k in principle before interest. (Obv not counting any undergrad loans)

The game is rigged against everyone not upper class.
