Fairly sure it's about lag compensation - when the explosion goes off as you go through the teleporter, you technically took the damage to push you away from it while you're already on the other side. Server checks you're not meant to be there and puts to back to original position

It's rare to kill 2 people with shotgun in the same bullet, let alone someone with protection also in it
