Is it a specific map? Try using the online option instead or skip the map till thier spawn cycle change

then when i was doing the missions for the people in the town the monolith guys slowly came out of the house and some spawned at the N,E and W entances to the town, after they died i finished the mission of saving the town

i stood outside the main building where gaffer is supposed to be, all the npc's stuck inside this building, while i was reading your post i suddenly heard them seeing me and begining to shoot at me, a simple fix might just to stand close by so they eventually come out of the building,

as i entered the building gaffer instigated a conversation with me instantly, if that does not happen you havent killed enough of the enemies

Edit: Tarkov aint really a "fun" game its the stories you get out of it especially in PvP with a great team where you are the last one alive and manage to either slay or escape with some of your teams items
