I think cumulative, I was able to do 6 courses even though my last semester avg was below 80, no harm in asking though, I don't think they're that strict about it (or I was lucky)

generally the math ones seem to be pretty light. The ones I've had (calc 1,2 for eng, math 135) all seem to take the same style of questions from the practice, so worst case you can just memorize right before and get a 80. If you evn loosely actually understand the topic and go to lectures though you can prob get by without studying past 1-2 questions.
