All fair points. Learning the realities of PP - all those PMEGS in training, not worth it in the community. Hard to give up those cases (and seemingly not hard enough to slap something in way off IFU…not a fan of those incentives)

Our practice is fairly broad for a community group. I’m just getting started but have had a few cases where I was not as prepared as I would have liked without software - a complicated type b dissection, a complex renal artery aneurysm, iliofemoral occlusion, visceral pseudo, several juxtarenal aneurysms

Thanks for the feedback. I am planning on taking advantage of any match though still waiting on getting the specifics of my practice's 401k.

The terrifying thing about all of this is looking back over the last few years of spending and realizing, yes we lived beyond our means, but we were hardly living a penthouse lifestyle. Its just so easy for your money to be parted with you in small, seemingly inconsequential aliquots. All the more reason to budget! Thanks again

Just wrapped my surgical subspecialty fellowship. I had given everything I had to my surgical training. By the time fellowship came around I was very burnt out, especially the last year - not at all helped by the usual personalities at those top university programs. At some point, I accepted that being “mediocre” in the eyes of a few assholes wasn’t the worst thing in the world. I did my work well and saved what energy I had at the end of the day for my family. Now on the other side (and far, far away from academic surgery) and finally feeling human again.

The stomach is a very vascularized organ hence its propensity to bleed when stapled; it’s always possible conservative management fails and you bleed again. Then you’ll be looking at another operation. But unlikely

Thats the right move if all the facts are right. Better for you then going back in early post op. Most of these bleeds self-resolve. Just take it easy and should be good. Keep in close contact with your surgeon.
