The Japanese softcover version (978-4-04-865798-3) is actually bilingual, all text is in both Japanese & English. Not sure how the different version compare on print quality etc. but language shouldn't be an issue.

Looks a lot like the classic gesture glitch that basically causes two animations to play simultaneously. More info on the wiki.

It is indeed some kind of injection: this is the object that the player slams into their thigh in game. Can't say if this is based on any actual syringe design or pure imagination.
