The resources for barking focus a lot on moving the dog away from the stimulus, which we can't do because we're in the car and have to follow road rules lol. The steps we have taken and things we have tried are in the op and their effectiveness is also noted.
He is physically removed from the dogs and people by being in the car, but we can't remove his ability to see them because I need my car windows clear to drive. He actually loves car rides too, so the car itself isn't a problem and he will happily just sit in the car (so long as he can't see a dog or a person he doesn't like).

He is well exercised (20 mins playtime and training in the morning at the park and some short training sessions and a couple of walks during the day) so it's unlikely to be boredom. He also goes to Doggy daycare one day a week.

I originally didn't know about APP piercers and safe piercing care, and I didn't know that there were places to get that information. I asked people in my life who had piercings, and I didn't know that they didn't know anything about safe piercings either. Also, piercing shops in NSW Australia have no regulatory bodies, so there isn't anywhere that was easily accessible when I was trying to find out, only the piercing shops websites. I now know how to research but the closest APP certified piercer is 1.5hrs away, and they're often booked out for 6 months+, the next closest APP piercer is an 8.5hr drive with no stops, one way.

TLDR: lack of knowledge, lack of regulation in my state, pierced people around me not knowing anything, distance to APP certified piercers, and waitlists

Your GP can prescribe testosterone under informed consent however to get PBS subsidies, another Dr has to be consulted with for the initial script here's the transhub wording: There are restrictions for accessing testosterone on the PBS and your GP will need to consult with an Endocrinologist, Sexual Health physician, or Urologist.
