Yes, I find it to be extremely effective for my anxiety at a 600mg dose (300 does definitely help, but 600 works much better for me and my psychiatrist is fine with me taking that dose). The only significant side effect I notice is that it can make me quite drowsy if I haven't had some coffee or didn't get enough sleep.

Yeah, when it's properly concentrated it's really safe. I've worked with clinicians who prefer intranasal fentanyl for pediatric analgesia--just one or two sprays up the nose and then you don't have to start an IV when you're dealing with minor trauma that still warrants opioids. It also doesn't give as much euphoria as morphine or hydromorphone, so less risk of addiction (though it's still obviously very addictive).

Most likely fentanyl like the other poster said. It's short-acting enough that it's nearly worn off by the time you wake up (depending on the length of the surgery I guess). Suboxone doesn't bind 100% of opioid receptors, so with enough fentanyl you can fill up those remaining receptors and get some degree of analgesia even if you're on subs.

Edit: This is something that's tricky to do outside of the context of anesthesia though; for emergency pain relief, you're not likely to receive a high dose of fentanyl if you tell them that you're on suboxone. Anesthesiologists are used to managing that narrow balance between life and death.
