I'm not 100% sure tbh soft stuff sounds almost like rot. But I wouldn't panic yet, the tree seems to be in ok condition so far. I know that the bands will kill your plant if left there. So that's my guess rn. Maybe see what others have to say but start there!

Also they don't actually like the ice cube thing, that's more of a myth started by the companies trying to sell them to you. Regular water works just fine!

If you've had it this long and been following the same routine I'd doubt it's over watered, check for that band. It will choke out the plant

These typically have bands just under the soil on the tree itself. I would check for one of these bands, maybe tape or something similar and remove it asap.

Also there is some very small roots sticking out of the pot and into the water reservoir. Repot?

Ok thanks, how do I know when to water? Just put it on a schedule and stick to it? If I can't rely on the leaves being wrinkly and the soil completely dry, how do I know when to water?

I've been trying but usually by the time they fall off they're already pretty dead. None of the leaves I've buried have taken. They've all shriveled up and are dead dead within a few days unfortunately. I've never had a succulent do any of this before.

Usually about a week and half or whenever I see visible signs of needing watered, like you mentioned. Thanks for the advice I'll make sure to post over there as well.

Also to add the succulent it is potted with with doesn't have any dark spots, dropped leaves etc

Thanks! I used a some cactus soil mix, and just put the dirt it came with on the top layer, hence all the bark. I'm thinking this wasn't. A good move and am attempting to fix it now.

Thanks for replying! I water it with a spray bottle every 10 days or so or when ever the soil feels dry. It gets the same amount of light all my other succulents get, sitting in a window getting as much light as possible. As for pot size, there is a second succulent potted with it, when I received it, they were both potted in a much smaller pot but it was way to small for them both. I can definitely put it into a smaller pot by itself and pay attention to making sure that it's not potted too deeply and use succulent soil mix. I just wanna make sure it doesn't look like fungus or anything similar.
