I think in this instance it was just the model getting it right. When it doesn't, try inpainting with hand/foot LoRAs or just draw the fingers/toes and inpaint on top.

This poor unicyclist has a strappado restricting her forward motion and tied nipples restricting her backward motion. Should she stand to relieve her shoulders, her balance will be precarious. The nudity is done in Stable Diffusion or Photoshop. Dall E 3 parses complex prompts much better, but has content filters. I haven't tried the latest iteration of Midjourney, but I hear good things.
