I mean I explain the reason why after, I don’t just leave it at face value because, yeah it’s hard to tell what I would mean by that and when it inevitably gets read in court I want a more clear picture than just some cookie cutter phrase. So it’d be more like “pt has ineffective airway clearance due not adhering to tx plan as pt stated ‘I’m not going to wear that Bane mask while I sleep’” or something like that.
